New looking for setup

Hello, years ago i used pansat and dreambox and would load keys and watch tv in usa. What boxes or setups work now? Do they need internet? I need a box that connects to satellite dish and give me usa premium channels. I do not want iptv as internet is not so good here in west coast. I am reading about oscam and ccam but i am not sure how it all works. Can someone take a brief second and tell me what is working. Thank you!
